For each pitch that we use we have a Risk Assessment sheet:

To accompany these documents is an Emergency Flow Document which contains information relating to emergency situations.

Emergency Flow Document

CHC Accident Form - to be filled in if an accident happens during training or during a match.

Also England Hockey has asked if there are any head or face injuries that they are recorded on the following form.
